Parents as Teachers Plus (PAT+) is an evidence-based home visiting program designed especially for pregnant women and families with infants up to age one who may be facing challenges including substance use concerns. Our compassionate team is here to support your journey from pregnancy through your child’s early years, with services continuing until your child turns three.

Unique Wrap-Around Care Just for You
What makes PAT+ special is our comprehensive approach to your family’s health. Our team includes a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) and a Mental Health Counselor working together to promote your family’s overall wellbeing. Your nurse helps establish your medical home and coordinates with your doctors, pediatrician, and birthing hospital staff. For expectant parents, we also offer valuable referrals to Doula services and Registered Dieticians.
